
Fees and payments?Fees are determined by the tooth being treated and the number of canals in the tooth. The fees for services rendered are the same charged to all patients for similar services, regardless of whether the patient is reimbursed by insurance. Payment is requested at the time of service. Visa, MasterCard and Discover are also accepted for your convenience. Insurance?If you do have dental insurance, we will file the necessary forms for you. Please bring the address and phone number of your insurance company so we can submit your insurance after treatment is completed. We ask that a minimum down payment of 30% of the total charge be paid before the insurance will be submitted. Please be advised that dental insurance benefits may be quite variable and are determined by the type of plan chosen by your employer. We are not involved with insurance carriers in any way. Since dental services are rendered directly to the patient, the patient is responsible for the total fee and will be expected to pay any balance due after the insurance payment. If there is an overpayment after the insurance carrier pays us, you will receive a prompt refund by mail.
